4. Documents required for disclosure requests
In order to respond to requests for disclosure (includes disclosure of records provided by third parties), etc. of personal information, after receiving the request, we will send the specified documents to the individual. Please fill in the necessary information on the documents and return them to the “Contact for requests for disclosure, etc.” above.

5. How to verify the identity of the applicant or their representative
Please enclose one of the following certificates and return it together with the required documents to verify the identity of the applicant. Please also take the time to mask the registered domicile on the driver’s license and passport so that the contents cannot be identified.
